Liability Waiver & Release of Claims

I, the undersigned, acknowledge that I am voluntarily participating in the Community Food Drop organized by St. Cloud Dream Center and URCHURCH LIFE Global Ministries, taking place on Monday, November 24, 2025, at BridgePrep Academy St. Cloud. I understand that participation may involve certain risks, including but not limited to physical activity, working around equipment, vehicles, and handling food supplies.

By signing below, I agree to the following:

1. Release of Liability

I hereby release, waive, and hold harmless St. Cloud Dream Center, URCHURCH LIFE Global Ministries, BridgePrep Academy St. Cloud, and all affiliated directors, officers, staff, volunteers, and partners from any and all liability, claims, demands, actions, or causes of action related to personal injury, illness, death, or property damage arising from or connected to my participation in this event.

2. Assumption of Risk

I voluntarily assume full responsibility for any risk of bodily injury, illness, death, or property damage that may occur as a result of participating—whether caused by negligence or otherwise.

3. Safety & Conduct

I agree to follow all safety instructions, event procedures, and guidelines, and to treat all volunteers, staff, and attendees with respect throughout the event.

Photo & Video Release

I grant permission to St. Cloud Dream Center and URCHURCH LIFE Global Ministries to capture photographs and/or video recordings of me during the event. I understand that these images may be used for promotional, educational, or informational purposes across various media, including websites, social platforms, printed materials, and reports.

I waive any right to inspect or approve the final use of these images and acknowledge that I hold no ownership rights or claims to the content.
