Liability Waiver and Release of Claims
I, the undersigned, hereby acknowledge that I am voluntarily participating in the Community Food Drop event organized by St. Cloud Dream Center and URCHURCH LIFE Global Ministries, held on Friday, July 11, 2025, at BridgePrep Academy St. Cloud. I understand and acknowledge that participation in this event may involve certain risks, including, but not limited to, physical activity, interaction with equipment, and working around vehicles or food supplies.
By signing this form, I agree to:

Release and hold harmless St. Cloud Dream Center, URCHURCH LIFE Global Ministries, BridgePrep Academy St. Cloud, and all of their affiliates, directors, officers, staff, volunteers, and partners from any and all liability, claims, demands, actions, or causes of action, including those for personal injury, illness, death, or property damage, arising out of or in connection with my participation in this event.

Assume full responsibility for any risk of bodily injury, illness, death, or property damage that may result from participation, whether caused by negligence or otherwise.

Agree to follow all safety instructions, event protocols, and respect all volunteers, staff, and attendees during the event.
Photo/Video Release
I grant permission to St. Cloud Dream Center and URCHURCH LIFE Global Ministries to take photographs and/or video recordings of me during the event. I understand that these images may be used for promotional, educational, or informational purposes, including but not limited to websites, social media, flyers, and reports, without compensation or further approval.
I waive any right to inspect or approve the use of the photograph/video and acknowledge that I have no ownership interest in the content.
